- HealthMedia® Breathe™ Smoking Cessation Program
- Breathe™ provides a customized, four-part action plan that is straightforward and easy to use, with a program length based on the participant's chosen quit date. After answering detailed questions about smoking-related issues, the participant receives a 16-page tailored action plan that identifies motivations to give up smoking, the factors favoring success, and the barriers to quitting. Most importantly, the plan gives the participant tailored strategies for overcoming barriers and effective steps for managing withdrawal. Three follow-up tailored action plans are delivered at the most critical relapse times-2 days, 10 days, and 21 days after the quit date. These follow-ups are designed to promote confidence and motivation, to increase active participation in the change process and to prevent relapse.
- HealthMedia® Balance™ Weight Management Program
- Balance™ is a 6-week program, which provides a customized, four-part action plan that is straightforward and easy to use. After answering detailed questions about weight-related issues, the participant receives a 16-page personally tailored action plan that offers techniques and strategies for making healthy food choices, increasing physical activity, and avoiding the emotional triggers that lead to overeating and self-sabotage. Three follow-up tailored action plans are delivered at one, three, and six weeks into the program. These follow-ups reinforce changes, deal with trouble spots, and provide additional support just when the participant needs it most.
- HealthMedia® Nourish™ Nutrition Program
- Nourish™ is an 8-week program, which provides a customized, four-part action plan that is straightforward and easy to use. After answering detailed questions about eating habits and behaviors, the participant receives a 16-page tailored action plan, which includes personalized techniques for making healthy choices when dining out, shopping, and preparing meals. Three follow-up tailored action plans are delivered at two, four, and eight weeks into the program. These follow-ups reinforce changes, deal with trouble spots, and provide additional support just when the participant needs it most.
- HealthMedia® Relax™ Stress Management Program
- Relax™ is a 5-week program, which provides a customized, four-part action plan that is straightforward and easy to use. After answering a series of in-depth questions about stress-related issues, the participant receives a 16-page tailored action plan, which includes personalized techniques for modifying behavior and attitudes related to stress management. Three follow-up tailored action plans are delivered at one, three, and five weeks into the program. These follow-ups reinforce changes, deal with trouble spots, and provide additional support just when the participant needs it most.
- HealthMedia® Care™ for Your Back
Care™ for Your Back is a program for self-management of back pain. Care for Your Back, complements existing disease management efforts to improve self-management skills and change the behavior that leads to chronic back pain, while being comprehensive enough to prevent back pain.

This program targets those employees who have the highest risks within your organization. AdvantageHealth health coaches work one-on-one with participants, empowering them to make long-term, healthy changes. The 6 month program includes monthly onsite meetings that build confidence and skills participants can use long past the end of the program. Each session consists of the following topic areas: readiness to change, short-term and long-term goals, motivation, triggers, coping skills and relapse prevention.

- Personalized Nutrition Consultations
- Using a 3-day dietary intake recall, each participant receives feedback from a nutrition professional regarding their food intake. Typical areas of discussion include: weight management, daily caloric/fat intake, food guide pyramid, serving sizes, behavior change, goal setting and motivation. Participants will also have the opportunity to ask specific questions regarding their eating habits, health history and goals for weight management.
- S.H.A.P.E. Weight Management Program
- This 12-week program has one goal: Support Healthy Eating And Promote Exercise! Through a cognitive and behavioral approach, S.H.A.P.E. focuses on increasing knowledge, coping mechanisms and weight management strategies for managing weight for a lifetime. A balanced diet following guidelines set by the American Dietetic Association and the United States Department of Agriculture is promoted. Regular aerobic exercise and strength training is also promoted following guidelines set by the American College of Sports Medicine. Each program is facilitated by a nutrition professional and includes up to a 60-minute weekly meeting consisting of voluntary individual weigh-ins, educational materials and group discussion on a variety of health-related topics. Participants complete a weekly food log which can be reviewed by the program facilitator on a weekly basis. One of our most requested programs!
- Smoking Cessation – Individualized Counseling
- One-on-one counseling sessions are an excellent way to assist smokers in their cessation efforts. With support and assistance from a tobacco cessation specialist, participants identify triggers to smoking, learn strategies to overcome urges, learn alternative behaviors and develop a personalized plan for quitting. The smoking cessation specialist is there for support and guidance along the road to quitting.
- Smoking Cessation - Support Group
- Support groups are an essential part of any behavior change program. Facilitated by a smoking cessation specialist, participants share their challenges and successes associated with their quitting process. The facilitator will continually provide additional resources and skills development along the quitting process. These groups typically meet weekly or bi-weekly for 45-60 minutes and are an excellent follow-up to individualized counseling or seminars.
How Does Telephonic Lifestyle Coaching Work?
Step 1: Eligible participants are determined from HRA data (typically those with 3+ risk factors).
Step 2: Health Coach invites via phone call. If participant is interested for the program, the process goes to Step 3. If participant declines, contact ends here, or a one-time health education call can be offered.
Step 3: Health Coach sets up month #1 call date/time during the invite call with the participant. Participants selects their module.
Step 4: (Months #1-6) Health Coach contacts participant at the agreed upon date/time each month.
Step 5: Upon completion of the program, employer group reports are provided detailing outcomes, participation, satisfaction and self-efficacy.
- Lifestyle Coaching
This program targets those employees who have the highest risks within your organization. AdvantageHealth has several direct mail pieces aimed to assist participants with combating their high-risk behaviors. There are over 25 health topics available and all include:
- Overview of the high-risk area
- Information participants need in order to identify their personal reasons for changing their high-risk area.
- Developing a personal plan for changing their risk area. A personal goal worksheet is utilized to keep track of progress and to stay focused.
- Details on motivation, relapse and the living without their high-risk behavior
- Behavior Change Brochures
- We can provide brochures that target specific health areas including: Healthy Diet, Physical Activity, Healthy Body Weight, Stress Management & Tobacco. The interactive brochures guide participants through exercises and activities specific to their “Stage of Change” and the health area they are most interested in.
